The WWIWCP is a collaboration between health system partners across the continuum in the Waterloo Wellington LHIN including the acute care, complex continuing care, rehabilitation, long-term care, community care and primary care settings.

Program Leadership

In support of an integrated approach to program leadership, the Waterloo Wellington Integrated Wound Care Council convened in January 2014 and is comprised of members from across the health care system including:

  • Primary Care (Family Doctors, Community Health Centres, Family Health Teams)
  • Community/Home Care (Care delivered through the CCAC)
  • Specialized Ambulatory Care Clinics
  • Acute Care Hospital
  • Rehabilitation and Complex Continuing Care Hospitals

Clinical Practice & Knowledge Translation Collaborative

As part of a two-year program action plan, the WWIWCP established a Clinical Practice & Knowledge Translation Collaborative to examine leading practices in the area of wound prevention and care. This group is comprised of a strong cross-sectional, multi-disciplinary panel of subject matter experts in wound management.
