The development and implementation of these resources is to help Health Care Providers to:

  • Find practical, evidence-based resources with which to care for individuals with or at risk of pressure ulcers
  • Perform a comprehensive patient assessment including assessing for pressure sore risk in the inpatient and outpatient care settings (Acute Care, Long Term Care and Community Care Settings)
  • Increase the use and implementation of pressure ulcer prevention plans for individuals at risk
  • Reduce the incidence of pressure ulcer development
  • Improve the frequency of skin inspections and re-inspections in patients/residents with identified pressure ulcer(s)
  • Perform accurate wound assessment including progress towards healing
  • Recognize signs & symptoms of infection and identify treatment intervention
  • Identify and implement appropriate topical wound care
  • Increase the use and implementation of evidence-based pressure ulcer treatment plans including pain management using pharmacological and non-pharmacological interventions
  • Educate regarding the prevention and progression of pressure ulcers to patients, families, and caregivers
  • Assist organizations in meeting the requirements of Accreditation Canada‚Äôs Required Organization Practices (ROP) for Pressure Ulcer Prevention programs, including the provision of education and measuring outcomes to determine effectiveness of the program, and to use results to improve the program
  • Improve the coordination and communication between care providers/care institutions regarding the transfer/discharge plan for patients with identified pressure ulcer(s)
